Description
Vitamin D3 functions as a secosteroid hormone precursor — after hepatic and renal hydroxylation to calcitriol, it binds the vitamin D receptor and regulates transcription across immune, skeletal, endocrine, and metabolic tissues. The 2,000 IU/day dose falls within the range studied in a landmark individual-participant-data meta-analysis of 25 RCTs (n = 10,933), which found vitamin D supplementation reduced the risk of acute respiratory tract infection (OR 0.88), with strongest effects in patients with baseline deficiency receiving daily dosing. The addition of vitamin K1 (250 mcg) and MK-7 (25 mcg) addresses the D3/K2 synergy rationale: vitamin D upregulates synthesis of vitamin-K–dependent proteins — including matrix Gla protein and osteocalcin — while vitamin K is required to gamma-carboxylate and activate them. MK-7 in particular has clinical data supporting improvement in arterial stiffness measures in postmenopausal women (Knapen et al., 2015 RDBPC trial). Dose note for practitioners: the MK-7 content (25 mcg) is lower than the 180 mcg/day used in hallmark arterial stiffness trials — clinical messaging should reflect foundational K2 support rather than therapeutic arterial calcification reversal.
Clinical indications include vitamin D insufficiency (particularly relevant in northern latitudes, indoor workers, and patients with malabsorption), immune support, calcium metabolism optimization, and patients on higher-dose vitamin D therapy who benefit from concurrent K supplementation to direct calcium appropriately. Key safety flag: vitamin K supplementation — including K1 at this dose — requires caution in patients on warfarin or other vitamin K antagonists; monitor INR if initiating or discontinuing.
