Adrenal Support is a morning formula. The six adaptogens I selected — KSM-66® ashwagandha, rhodiola, eleuthero, shatavari, holy basil, and tinospora — work through distinct mechanisms to support cortisol regulation, energy, and stress resilience during waking hours. KSM-66® is the most clinically studied ashwagandha fraction, with multiple double-blind trials showing statistically significant cortisol reduction and improvement in stress and fatigue scores. Rhodiola addresses the mental fatigue and burnout presentation through a separate mechanism. Eleuthero supports non-specific stress resistance via the HPA-immune interface. Holy basil modulates cortisol and inflammatory signaling through COX pathway activity. Shatavari bridges the adrenal and reproductive axes — particularly relevant for the perimenopausal patient. Tinospora adds immunoadaptogenic activity that matters when chronic stress has been suppressing immune tone. The cofactors aren’t filler: adrenal glands hold the highest concentration of vitamin C in the body, cortisol synthesis runs through the B5 pathway, and I used active-form B6 (P5P) and B2 (riboflavin-5-phosphate) because a significant share of this population cannot efficiently convert dietary B vitamin forms.
I use this most with patients who are functionally managing but running on empty — perimenopausal, post-burnout, flat-line morning cortisol, fatigue that sleep doesn’t resolve. Because this formula is built for daytime energy and resilience rather than sedation, it works alongside Adrenal Calm rather than instead of it. Adrenal Calm handles the evening side of the HPA axis picture — the patients who can’t wind down, sleep onset difficulty, the nervous system that never fully downregulates. Many of my patients do well on both together.
